Employees can access these forms by logging into my.fordham.edu, click on Employee, then Electronic Forms. For your reference:
- Access the Employee Personal Change Form for updates to your name, address, phone number, etc.
- Access the Employee Action Form for new assignments/payments, changes to existing assignments, or to update budget and supervisor information.
